PUPPIES:  
5-in1 and Corona are given at 8, 12, & 16 weeks of age and then annually thereafter. 
Rabies are given at 16 weeks of age and must be repeated one year later.  Colorado:  every 3 years thereafter UTAH:  every 2 years thereafter.  
Bordatella & Lyme requires 2 vaccinations given 4 weeks apart and then annual thereafter.
Deworming:  Given orally and to be repeated 10 days later.  Veterinarian will prepare a syringe for the 2nd dosage to be given in 10 days.

KITTENS: 
4-in1 and FELV are given at 8, 12,  weeks of age and then annually thereafter.
